Blood Flow Restriction training, often shortened to BFR, is a training method where an inflatable cuff is placed around an arm or leg to partially restrict blood flow while exercising with a light load. The goal is to achieve strength and muscle gains comparable to heavy training, without the heavy load on joints and tissue.

How does it work?

The cuff is placed around the upper arm or upper thigh and inflated to a moderate, controlled pressure – enough to restrict venous return (the blood flowing from the muscle back to the heart), but without cutting off blood supply into the muscle. You then train with low resistance, typically 20–30% of what you could lift once at maximum.

This combination causes a local build-up of metabolic by-products in the muscle faster than usual, which triggers many of the same signalling pathways for muscle growth and strength normally seen with heavy strength training.

Why is this useful in rehabilitation?

After surgery or a more serious injury, it's often neither possible nor advisable to load heavily. At the same time, muscles lose strength quickly with inactivity. BFR training makes it possible to stimulate the muscle effectively with light weights – something that is far gentler on a joint or tendon that's still healing.

The method is therefore often used early in rehabilitation after surgery (such as knee surgery), for more serious injuries where heavy loading isn't yet advisable, or when pain limits how heavily you can train.

Is it safe?

Used correctly, with the right pressure and under guidance, BFR training is considered safe for most people. A thorough assessment beforehand is still important, as the method isn't suitable for everyone – including those with certain vascular or heart conditions, risk of blood clots, or uncontrolled high blood pressure. Pressure and duration should always be individually tailored, not standardised.

Who is it suitable for?

  • Patients early in rehabilitation after knee or hip surgery
  • Athletes wanting to maintain muscle mass and strength during an injury period
  • Those who, for various reasons, can't tolerate heavy loading on a joint or tendon
  • Older adults or others seeking strength benefits without the mechanical load of heavy training

How I work

I consider BFR training as one of several tools in individually tailored rehabilitation programmes, particularly when load tolerance in an injured tissue is limited. The decision is always based on a thorough assessment of you and your specific situation.
